Chronic Disease Management

Chronic diseases are medical issues which become prolonged or lifelong illnesses.  Examples include diabetes, high blood pressure, depression, asthma, arthritis, and many others.  These illnesses require more frequent monitoring and follow up visits with your medical provider.

Many patients are taking prescription medications which require regular monitoring of bloodwork to ensure your body is safely handling these medications.  Regular visits with your provider allows for evaluation of your treatment plan, addressing your questions and concerns and avoiding medical errors.  We recommend that any patient on regular prescription medications bring all medication bottles with them to their visits, or at least an updated list of exactly which medications they are taking, including all vitamins and herbs.
