Once you have an appointment, you will receive an email from Docusign with your telehealth waiver. Please electronically sign and return the waiver ASAP before your appointment.
Please have the following ready at the time of your visit.
- Today’s weight
- Blood Pressure (if you have a b/p cuff at home or a smart device)
- Pulse (if you know how to take this or can obtain this using a smart device)
- A list of your current medications and the dosage you are taking.
- Glucose level for diabetics
Once you have an appointment, use the links below to enter the virtual waiting room for your provider. Then just wait for your appointment to start.
